American Traveler is seeking an RN with home care and case management experience for a Home Health float position in the South Region with active MA licensure required.

Details

• Work in a home health setting serving the South Region, including multiple towns
• Float position covering Abington, Bridgewater, Brockton, Carver, Cohasset, and additional nearby communities
• Patient population ranges from infants to seniors over 100 years old
• Day shift hours are 08:00-16:30
• Uses Epic EMR for charting
• Weekend rotation required every six weeks
• Holiday and on-call coverage according to departmental rotation

Requirements

• Active, unrestricted RN license in Massachusetts; conditional or pending licenses are not accepted
• Minimum 1 year of certified home care experience
• Minimum 1 year of case management experience
• Valid driver's license and reliable independent transportation for patient visits
• AHA certifications required
• Epic EMR experience preferred
• Proof of flu vaccination required; no exemptions allowed
• Primary source verification of professional license and certifications required
• References required for consideration
• Local candidates must reside at least 60 miles from Massachusetts General Hospital in Boston

Additional Information

• Responsible for patient assessment, care plan implementation, and ongoing education and support for patients and families
• Duties include admissions process and case management in compliance with organizational policies
• Must utilize thorough and timely electronic documentation and interdisciplinary communication
• Parking at most locations may require use of public lots and may incur a daily fee
• UKG/Kronos is used for timekeeping
• Flu vaccination is mandatory during flu season
