Description

American Traveler is seeking an experienced RN Case Manager for an adult medicine/surgery inpatient case management position with day shifts and Epic EMR experience required.

Details

• Work in an inpatient hospital setting on the Adult Medicine/Surgery Case Management unit
• Day shift schedule: five 8-hour shifts, 8:30 AM to 5:00 PM
• No weekend, holiday, or on-call requirements
• Patient caseload involves transition planning for adults needing post-acute care
• Uses Epic EMR and referral platforms such as NaviHealth or AllScripts

Requirements

• Active RN license (MD or compact)
• Minimum two years of RN experience
• Current Basic Life Support (BLS) certification
• Current inpatient case management experience required
• Experience in facilities with 500+ bed counts required
• Experience with Epic and post-acute referral platforms such as NaviHealth or AllScripts

Additional Information

• Coordinate care and transition planning from triage to discharge for adult surgical and medical patients
• Work collaboratively within a multidisciplinary team and communicate with payors to secure approval for treatment plans and resources
• Manage patient information in departmental systems according to operational guidelines
• Must hold a current CPR card if performing bedside case management
• No medication administration or med testing required for this position
