Details

Work in the Case Management department of a hospital setting
Patient population includes adolescents, adults, and geriatrics
Day shift schedule with four 10-hour shifts per week (8:00–18:30)
Required to work two weekend shifts every four weeks
Utilizes Epic for charting; strong Epic proficiency required

Requirements

Active CA RN license required
Bachelor of Science in Nursing (BSN) required
Basic Life Support (BCLS) certification required
Minimum of two years recent Case Management experience required
Strong acute care experience required
Strong Epic charting experience required

Additional Information

Responsible for determining appropriateness of hospital admissions and coordinating interdisciplinary care plans
Promotes efficient utilization of resources for optimal patient outcomes
