American Traveler is hiring an experienced RN Care Coordinator for an acute care case management/utilization review role requiring a current CA RN license and recent Dignity Health Care Coordinator experience.

Details

• Work as a Care Coordinator in an acute care hospital setting
• Case management and utilization review with a diverse patient population including cardiac, surgical, oncology, and critical care cases
• Manage a daily caseload of 15-25 patients, with possibility of higher volume due to staffing
• Float to all units except surgery as needed, with priority for floating given to travelers
• 8-hour day shifts with every other weekend required
• Uses Cerner EMR system

Requirements

• Active CA RN license required at the time of consideration
• Current BLS certification required
• Minimum 2 years of recent RN experience
• Recent experience as a Care Coordinator at a California Dignity Health Hospital is required
• Experience in care coordination/case management in acute care setting
• Two professional references required: one from a supervisor in the last 12 months and one from a peer or supervisor in the past 3 years
• Copy of driver's license required for consideration

Additional Information

• Conduct care coordination and utilization review for a variety of complex patient cases
• One day of orientation provided with an RN Care Coordinator
• Must pass Relias Dysrhythmia exam and sterile technique competency during orientation
• Candidates may not have been directly employed by CommonSpirit, CHI, or Dignity in the past year unless PRN through outside firms
• First to float among staff as needed
• Dress code and additional requirements to be confirmed during orientation
