American Traveler is seeking an experienced RN Float for a Home Health assignment in the Boston area requiring an active MA RN license and strong case management skills.

Details

• Home health care position floating across multiple neighborhoods in the North Region of Boston including Back Bay, Beacon Hill, Cambridge, Charlestown, and more
• Day shifts from 08:00 to 16:30
• 13-week assignment
• Patient caseload includes adult community health populations requiring diverse levels of care
• Must be able to travel to cases as assigned across designated service areas
• Weekend rotation is required once every 6 weeks
• Documentation is completed using Epic EMR; experience with Epic preferred

Requirements

• Active MA RN license required; no conditional or pending licenses accepted
• Minimum 1 year of certified home care experience
• At least 1 year of case management experience
• Certifications must be American Heart Association (AHA) only
• Proof of MA RN licensure required for consideration
• Flu vaccine required with no medical or religious exemptions accepted

Additional Information

• Responsible for patient assessment, implementing care plans, providing ongoing education, and managing case admissions
• Must adhere to organizational standards of community health practice and timely electronic documentation
• Expected to participate in interdisciplinary communication and collaborate on patient care plans
• Parking is not validated at Boston locations; public parking or transportation recommended
• First-time travelers are accepted for this position
• Local candidates must have a permanent residence at least 60 miles from Massachusetts General Hospital
• Scrub or professional attire required as per home health dress code
