American Traveler is seeking an experienced RN Patient Care Coordinator with a current OR or WA license and at least one year of case management experience.

Details

• Work in a patient care coordinator role within a hospital setting
• Standard daytime shift from 8:00 AM to 5:00 PM, 40 hours per week
• Weekend and holiday shifts required
• Utilizes EPIC for charting and documentation

Requirements

• Active RN license in OR, WA, or compact state required at time of consideration
• Minimum of one year of case management or patient care coordinator experience
• Experience with Medicaid/Medicare processes preferred
• EPIC EMR experience required
• Previous experience with Kaiser Permanente preferred
• Must provide last 4 digits of SSN and date of birth for consideration

Additional Information

• Responsibilities include coordinating patient care and supporting case management activities
• Must be able to work weekends and holidays as scheduled
• Both local and travel candidates are accepted at the same compensation rate
• Candidates cannot exceed two years of assignment time within the health system without a 90-day break
• References required at time of consideration
