Description

American Traveler is seeking an experienced LPN for a home health position requiring OASIS experience, IV competency, and the ability to manage high-risk diagnoses.

Details

• Work is performed in a home health setting with Medicare, Medicare Advantage, and commercial insurance patients
• Patient caseload includes 6-8 home skilled visits per day
• Frequent conditions managed include heart failure, COPD, MI, pneumonia, and stroke
• Procedures include wound vac dressing, Pleurx/Asept drain care, foley catheter changes, staple and suture removal, glucometer use, lab draws, IV skills, PICC line or Midline care, and SQ port access/deaccess
• Schedule consists of varied day shifts, including Mon-Fri 8a-5p or 9a-6p, and may include Thurs-Mon or Fri-Tues shifts
• Alternating weekend shifts may be required
• Nurses use BayCare Scheduling, Ace Timesheet documentation, laptops, MiFi for internet, and portable printers

Requirements

• Current LPN or LVN license required if applicable
• Minimum 1 year of skilled home care experience required; private duty experience alone is not accepted
• OASIS training and documented experience is required
• Current BCLS certification required
• Strong IV and wound care skills required
• Valid driver's license and personal vehicle required for home visits
• COVID-19 vaccination or approved exemption required
• Annual flu shot or signed declination with mandatory mask use required while working

Additional Information

• Home health responsibilities include cardiopulmonary assessments, wound care, SQ/IM injections, lab draws, medication administration, and post-op care
• Self-scheduling of visits and hours is expected based on HomeCare office needs
• Any solid color scrubs are acceptable
• Laptops, mobile internet, and portable printers are provided for documentation and scheduling
• Locals are accepted at the same rate
