American Traveler is seeking an experienced RN for a Home Health/Hospice assignment serving a primarily geriatric population, requiring a WI or compact RN license and experience in chronic disease management.

Details

• Work in a home health and hospice setting caring for geriatric patients in a post-acute environment
• Patient population includes individuals with chronic and complex medical conditions such as heart disease, COPD, dementia, cancer, and surgical recovery
• Typical daily caseload is 5-6 patient visits with a weekly expectation of 25-30 points including both routine and OASIS visits
• Utilizes Cerner EMR, transitioning to Epic
• Monday through Friday schedule, 8:00 am to 4:30 pm, with end time subject to change based on patient needs
• Weekend work required only during assigned on-call periods
• On-call rotation is one week/weekend every six weeks (non-consecutive days)
• No floating required, but both home health and hospice visits are expected

Requirements

• Active WI or compact RN license must be active by assignment start date
• Current BLS (AHA) certification required
• Minimum of 1 year of recent RN experience in home health or hospice
• Experience in travel nursing, critical thinking, chronic disease management, patient and family education, negative pressure wound therapy (NPWT), ostomy care, central line care, IV administration, and complex wound care required
• Hospice experience including pain and symptom management, end-of-life care, and demonstration of empathy and compassion
• Experience with Cerner EMR preferred
• Two professional references required, including one supervisor within the past year and one peer or supervisor in the last three years of relevant experience
• Former permanent staff must have been separated from the facility for at least 6 months

Additional Information

• Daily responsibilities include home and hospice visits, medication management, wound care, patient and family education, and participation in multidisciplinary huddles and meetings
• Participate in a daily 15-minute team huddle, weekly interdisciplinary team meeting, and monthly all-staff meeting
• Field training and orientation range from 2-5 days depending on experience, with an additional day of computer-based training
• Mentorship and support available from certified nursing assistants and an administrator accessible after hours, weekends, and holidays
• Scrub attire must be solid colors with closed-toe, non-slip shoes; no patterns or non-company logos permitted
• Must be available to work at least one major and one minor holiday during the assignment
• Local candidates within 80 miles of the facility will be considered at a reduced pay rate
