American Traveler is seeking an experienced RN Case Manager with acute care background and a WV or compact license for a weekday role focused on discharge planning and care coordination.

Details

• Work in a hospital-based case management unit covering 35-40 patients per day
• Day shift schedule: 5x8 hour shifts, Monday through Friday, 8:00 am – 4:30 pm
• Every other weekend required
• Orientation is held every other week on Thursday
• Floating to other units within scope may be required to meet facility needs
• Patient population includes diverse acute care cases in a Level 1 trauma, major teaching hospital setting
• Uses Cerner EMR; experience preferred but not required
• Business casual dress code with white lab coat or jacket required when on the floor
• Free parking provided for travelers

Requirements

• Active WV or compact RN license required; may submit with pending license
• Minimum 2 years of acute care RN experience required
• Current BLS certification required
• Experience in discharge planning and care coordination preferred
• Case management experience in an acute hospital setting strongly preferred
• Familiarity with MCG (Milliman Care Guidelines) is a plus
• Must provide at least two manager or charge references from the past 2 years
• Must reside at least 100 miles from the facility; candidates between 75 and 100 miles considered for approval
• Flu shot required during October 1st to March 31st unless medically exempt
• Previous permanent employees must wait 2 years before being eligible for a travel contract
• Not eligible for current Vandalia Health System employees occupying a permanent, PRN, or other employee role

Additional Information

• Primary responsibilities include discharge planning and care coordination requiring real-time documentation
• May be required to request authorizations for post-acute placement or discharge medications
• Experience with risk assessments and addressing SDOH is advantageous
• Modules must be completed prior to start, with certificates provided upon completion
