American Traveler is hiring an experienced RN Case Manager with an active UT or compact RN license and acute care case management experience.

Details

• Position is in a hospital setting, focused on case management and discharge planning
• Provides coordination of care from hospital admission through post-discharge in both inpatient and outpatient settings
• Day shift schedule with 5x8 hour shifts, 8:00 am to 4:30 pm
• Works collaboratively with internal teams, payers, vendors, patients, and families

Requirements

• Active UT or compact RN license required prior to start (pending accepted at time of consideration)
• BLS certification required
• Minimum 1 year of prior case management experience in an acute care setting
• Candidates must reside at least 100 miles from the facility; local candidates are not accepted
• Previous full-time employees at this facility are not eligible for this travel position

Additional Information

• Responsible for monitoring patient progress, resolving care obstacles, and developing and executing individualized discharge plans
• Participation in orientation modules required on the first day
