American Traveler is seeking an experienced RN for a Home Health position in Texas requiring HCHB and OASIS experience, with on-call and travel requirements.

Details

• Work in a home health field setting providing care in patient homes
• Manage a caseload of 4-6 patients per day, with a minimum of 6 points daily and 30 points per week
• Patients include geriatric and diverse population groups
• Homecare Homebase (HCHB) and OASIS charting systems are used and required at the point of care
• Schedule is Monday through Friday, 8:00 am to 5:00 pm
• Weekend and on-call rotation required, including at least one night of call per week
• Must be willing to travel up to 50 miles from home base; local candidates accepted within a 50-mile radius
• Mileage reimbursed at IRS standard rate from first to last patient visit

Requirements

• Active TX or Compact RN license in hand
• Current BLS certification required
• Minimum 1 year of home health RN experience and 2 years total RN experience in a clinical setting
• Proficiency in HCHB and OASIS required; HCHB training certificate must be provided if not previously trained
• Current driver's license, reliable transportation, and auto insurance required
• Must be a non-smoker due to patient sensitivities
• Proof of current CPR certification required
• Candidates with hits on license will not be considered

Additional Information

• Main duties include comprehensive in-home assessments, Plan of Care development, and coordination with interdisciplinary team members
• Must complete all charting while at the patient's home
• First-time travelers are accepted for this position
• Scrubs of any color are allowed for dress code
• CHPN certification preferred but not required
• Must provide interview availability at time of application
• Proof of HCHB training must be submitted prior to start; candidate is responsible for training cost if not already certified
• No RTO (Requested Time Off) will be accepted after application submission
