American Traveler is seeking an experienced RN Case Manager for a hospital-based role serving adult and geriatric patients requiring acute care case management and discharge planning expertise.

Details

• Work in a hospital case management unit
• Adult and geriatric patient population with a typical caseload of 25-30 patients
• Day shift, 5x8 hour days, 8:30am-5:00pm Monday through Friday
• One full weekend shift required every 4 weeks
• No on-call or holiday duties
• Utilizes Cerner, CarePort, AllScripts, Powerchart, and Epic EMRs

Requirements

• Active FL or Compact RN license in hand
• Minimum 2 years recent consecutive acute care hospital case management and discharge planning experience within the last 24 months in a US hospital
• Degree from an accredited nursing program or approved by FBON
• Experience with Level 1 Trauma preferred
• Proficiency in Microsoft Office Suite
• Experience using Cerner, CarePort, AllScripts, Powerchart, and Epic required
• Must pass EKG and arrhythmia exam per testing standards
• Proof of current certifications required for consideration
• No degrees from Operation Nightingale schools accepted

Additional Information

• Initial patient assessment required with follow-up visits at least every third day

• Will need to complete pre-start modules and provide self-submitted certificates of completion
• Local candidates within 60 miles considered at a different pay rate
• Former permanent employees must be separated for at least 2 years before being eligible as a traveler
• Must provide proof of vaccination status with no declinations allowed
