Job Details
- Work is conducted in a home health setting in the Kāne‘ohe area of Hawaii
- Dayshift schedule with 8-hour shifts and a 40-hour work week
- Weekend work is on a rotating basis, typically 2-3 per month
- No on-call responsibilities
- Requires use of Homecare Home Base and OASIS documentation
Job Requirements
- Active Hawaii RN license required
- Minimum of 2 years recent Home Health RN experience required
- Previous travel nursing experience is required
- Valid driver's license and auto insurance required
- Proficiency with Homecare Home Base and OASIS is required
- COVID-19 and flu vaccination are required; declinations are not accepted
- Bachelor’s degree preferred
- Case manager experience preferred
Additional Information
- Coordinates patient care and collaborates with physicians and multidisciplinary teams to optimize outcomes
- Acts as a consultant to the healthcare team and serves as a liaison for patients and families
- Primary focus on complex cases, high ED utilizers, chronic pain, substance use, and homeless patients
- Handles patient transfers to tertiary centers and ensures continuity of care
- Oversight of compliance data and frequent emergency department admission management
- Mileage is reimbursed at $0.43 per mile
- No more than 5 days of requested time off (RTO) will be considered
